Sharing Thoughts:

It was good time to share with some of our friends last month, who were on their visit of India and visited my clinic and also spare their valuable time and participated in ayurvedic activities like lectures etc.

I wish to thank them all; Dr. N.G. Koastopoulos (Greece), Dr. Power (Irland), Dr. Ashvin Barot (UK), Dr. C. Monsanto (Holland), Mr. Melnikov (Ukraine), Mr. Prafull Patek (UK).  - Dr. Prerak Shah, Editor, Ayulink

Problem of the month: (through e-mail counseling)

From my personal communication with a leading scientist of USA about
Fibromyalgia: (Myofascial Pain Syndrome: Fibromyositis)

It is non-articular rheumatic disorder with pain, tenderness and stiffness of muscles. Symptoms include:

MAJOR CRITERIA:

1.  Generalized aches or stiffness of many anatomical sites for at least three months

2.  Six or more tender points

3.Eliminate other disorders which can be diagnosed to be specific but different

MINOR CRITERIA:

1.  Generalized fatigue

2.  Chronic headache including migraines

3.  Sleep disorder, particularly timings of cycles

4.  Neuronal and psychological complaints

5.  Swelling

6.  Numbness/ tingling sensations

7.  Irritable bowel syndrome

8.  Stress, and weather changes

Fibromyalgia is a recently recognized dis-order that is regarded as a common cause of chronic musculoskeletal pain and fatigue. Fibromyalgia is a relatively common condition, estimated to affect about four percent of the general population. Very similar to chronic fatigue syn-drome (CFS); The main difference is musculoskeletal pain in fibromyalgia and fa-tigue in CFS. Low level of Serotonin is considered to be most important biochemical criteria. The primary treatment goals should include raise serotonin levels, improve sleep quality, and assure adequate magnesium levels. So from Ayurvedic point of view we must consider sleep patterns, particularly sleep and migraines.

Sleep has 4 non-REM (Rapid eye movement) followed by REM. In fibromyalgia REM is decreased and Non REM is increased. . So, when you wake up you feel tired and worn out. Similar to CFS. But now you have pain along with it. One must focus on Serotonin (5 HT -amine, and precursor 5HTP, 5 -hydroxytryptophan).

Of course sleep and Melatonin  (two step conversion of 5-HT) are also involved.  The site of synthesis is brain, specifically superchiasmatic nucleus and pitutary. So consideration of Panchkarma wold be a good therapy, particularly Pichu.

Oral intake of 5-HTP has definitely good response in 50%, fair in 35%, poor in 15 % cases. If we provide directly by infusion (Pichu) it may prove very beneficial. I plan to consider this approach. We may also consider (from your experience) specific medicated oils/ghrita. If I know what might be the ingredients, then we may check if they contain either 5HT/5HTP or biogenic mines? We must also consider dietary requirements.

Ayurvedic plants contain biogenic amines, specifically those mentioned above. Herbal and other treatments include St. John's wrote extract (0.3% hyperi-cin): 300 mg three times per day, multi-vitamins and minerals, 5-HTP:

50-100 mg three times per day. Also, the role of magnesium is considered important. How to include this salt -Kshar/ Bhasma requirement is also important. My Molecular Meditation will also contribute t activate specific sites in the brain.

Note: My earlier research showed (specific cells in insect brains) produced 5 HT, which worked at fempto gram level in inducing salivation, by regulating cyclic AMP, a very crucial pathway for transport. I strongly believe that most all Panchkarma treatments must activate this pathway. Salivation is also a problem with Fibromyalgia patients. Everything points to Brain and biogenic amines, and Panchkarma would be a very good delivery vehicle.
